Kitchener Office Closure

Dear Valued Patients,We sincerely regret to inform you that Mango Tree Family Health Team, Waterloo office is being forcibly evicted without notice from the Eastbridge Medical Clinic. This mandatory closure means that our Waterloo clinic will be closing July 30th, 2024.The Mango Tree team remains committed to your health and well-being despite this unexpected event. Our Guelph location remains open. We are also offering the following services to ensure you continue to receive care as you transition to the Guelph location or transition to a new medical care provider.

1. Ongoing Care Provision: Our Guelph location will continue to provide medical care and

   
   

   will accept Kitchener – Waterloo patients. The Guelph clinic is located at 28 Brock Rd N,

   
   

   Guelph N1H 6H9. For patients unable to travel to Guelph we will continue to provide

   
   

   virtual care as appropriate for a period of 3 months as you transition to a new primary

   
   

   care provider.

2. Continued Staffed Phone Lines: Our phone lines will remain operational during normal

   
   

   business hours to assist you with any queries or concerns.

3. Referral to Other Physicians: We recommend contacting Health Care Ontario for

   
   

   assistance in finding a new primary care provider. To register by Phone, dial 811 or go to

4. Chart Retention: Your medical records will be securely stored for ten years, and we are

   
   

   more than willing to transfer them to your new healthcare provider upon request.

5. Medical Emergencies: In case of a medical emergency, please proceed to the nearest

   
   

   hospital emergency department or call 911.
